HCHS Final Forms

Parents and student athletes must register at Hertford County  Final Forms to comply with all regulations for playing extracurricular athletics at HCHS.  Parents are required to register their students before student accounts can be created.  Parent accounts must be created using a personal email and cannot use an HCPS student email account in order for parents to have full access.   If you have forgotten the password or email address, use the Forgot Email or Forgot Password Recovery system on the home page of Final Forms.  Do not create a new account. 

Registration for sports begins on June 1; all documentation for fall sports must be entered no later than August 31st; for winter sports no later than November 21st and for spring sports no later than January 31st.  It is the responsibility of parents and student athletes to ensure that all documentation has been submitted to the Athletic Office prior to the start of the season in which the student wishes to participate.  

NCHSAA CrashCourse Concussion Video Requirement

Complete the NCHSAA required concussion training here: CrashCourse.  Completing the form will submit your information to the database.
